Electoral roll issue, account refused.

Some time ago my girlfriend tried to open a bank account with the Co-Op but got refused. (Basic Account, Well thats what she asked for)
She thought nothing more of it until recently when she got her equifax credit report. All clear obviously as she has had zero credit history.
But a discrepency with her name on the electoral roll.
Seems they had my surname down instead of hers. She has corrected that but it now shows her as living at the address for 1 year and not the full time she lived there, Approx 10 years.
So if she tried to open an account she would say 1 year and ask for previous addresses where there are none? Or say 10 years and they see she has not lived there? I guess this seems dodgy to them?
I did ask equifax but they were useless. Its now been over 2 weeks since they last replied to her question on how to correct this.
Strange because the council tax came in joint names, Correctly addressed. Only the voting card had the wrong surname so we assumed it was OK.
Any suggestions?
PS.. Equifax.. OH MY GOD.... USELESS....
They asked for ID. Driving Licence or Passport. She doesnt have either.
What else. Then they say the 2nd form of ID.. STOP.. What can she use for the 1st form form of ID if she does not have a passport or driving licence. err err..Lets carry on with the 2nd form of ID she says.
And now they have stopped answering questions.
Even though it was only £2. They are not worth it.

Equifax have done nothing wrong about the data being recorded that way. The gripe with them is the initial contact to actually get the credit report and asking for ID she did not have. And could not understand someone not having a passport or driving licence. And took several attempts and had to call back to get someone different after she repeated the same 2 forms of ID over and over.
The 2nd person started with the same 2 forms of ID then eventually went away & came back with a Birth Certificate instead. 17 minutes and 2 calls..
Then the lack of response when asking howto correct this information.
She assumed the data was for the voting card only, Had no clue as to it being data that applied to her credit file. She never had credit.
No bank account, No loans etc etc etc.
Job paid cash weekly so she had no need for them.
So the big question is how can she correct this now?

Which is strange as the voting cards were probably addressed and proper name when she moved in?

OK just had a strange thought. Could it be possible there is a separate credit file attached to the wrong name?
