I am confused by your post and not sure if you are the person receiving HRM or if you want to get a car via someone else's HRM.
I think the 5 mile rule was brought in to try to minimise mis-use of the scheme. The car must be used for the benefit of the person with HRM.
It is possible to have a nominated driver that lives further than 5 miles away. I spoke to Motability yesterday who briefly mentioned there is a procedure that you can go through and if successful then the nominated driver can live further away. I assume not everyone who requests this will be automatically successful and I am not sure what the procedure involves as I have not yet gone through it.0
