How to pay off debt

Help!
I was diagnosed with cancer toward the end of last year, and have had months of intensive treatment. This has thrown my finances into a complete mess - I am in hospital a lot, and find it hard to keep track of things, and also am not able to work quite a bit of the time. I have cc debts of about 12k, which I could afford before, but have now got into arrears, and am finding it a struggle to meet the monthly repayments. I won't be able to take out a loan because of my illness, and besides, my credit rating has now been damaged.
I want to start paying a manageable amount each month with a view to clearing the card and closing the account. My illness is looking like I will have a long term fight on my hands - so I need to sort something out. Will the cc company allow me to make smaller payments on a long term basis?
All advice gratefully received....
