MSE Write to Lender Template HELP!?

Hello there,
I'm soooo sorry if I am writing this in the wrong place, but I am new to this forum (1st post) lol.
I've been researching PPI claiming yourself on here for a while and today I decided to d/l the MSE letter template.
Upon doing so, everytime I click on the 'write to your lender' template (d/l link), it is taking/opening the PPI Consumer Questionnaire (d/l link instead) that you send to the Ombudsman.
Help pleeeeeeease?

Hi and welcome
The Financial ombudsman service (FOS) issued these reclaim questionnaires now as well to use to complain to the lender, you keep a copy and if you need to complain to the FOS later on, you then send them a copy to save time later on.
The template letter to reclaim was replaced by the questionnaire.;)

Sorry I got a bit confused as it said in the 'what to do next section' to 'return to the address at the end'.

You initially send the completed Ombudsman's Questionnaire to your lender. If your lender refuses to uphold your complaint , you then send a copy of the same Questionnaire to FOS.
