Thames Water Build Over Agreement

Does anyone have any examples of what Thames Water ask for as part of a build over agreement. I live in Richmond and am in the process of planning an extension, we have a manhole in our existing conservatory that will need to be sealed and moved so that rodding access is provided away from the conservatory.
Does anyone have any examples of what Thames Water may require?
We have a mid terraced house and are mid run on the sewer run.
