Arabic to english translation needed for insurance

Hey all, new here.
I recently had my phone stolen while in Egypt. Shortly after I went to the local station and got a report form for insurance.
I was just wondering whether there is anybody that can speak both arabic and english that is able to translate the 4 lines or so of written text on the form, as I'm not sure exactly what details they have included and what they have omitted, so I would like to know this prior to filing the claim.
Possibly a bit of a long shot, but if anybody is in a position to help it would be great if you could shoot me a PM.

If you don't know anyone with Arabic language skills, see if your local Mosque will help. Don't go on a Friday, as that is their main day of prayers.The comments I post are personal opinion. Always refer to official information sources before relying on internet forums. If you have a problem with any organisation, enter into their official complaints process at the earliest opportunity, as sometimes complaints have to be started within a certain time frame.0 -

Good idea.
Also, depending on where you live, takeaway shops and fast food restaurants may turn up an Arabic speaker or two
(two of my regular takeaway spots are staffed by Iraqis).
Any Lebanese restaurants? Sure there's someone who'll do 5 mins work for a small tip
There's loads of different dialects of Arabic, but if it's written in Egyptian Arabic, most Arabs will understand it fine, it being the dialect of movies, music, etc.0
