Is there a PPI policy no. for a Credit Card?

Hi,
I'm just getting my information together for a claim against HSBC for misselling PPI on my Visa card. I don't have the original documents though, as it was taken out several years ago.
But I don't ever recall any separate policy document with a policy number on it. I'm sure it was just a tick-box on the original application and then the amount charged gets added to your balance every month.
So what do you put in the policy number section? Can it be left blank?

I also just found a few random posts where ppl had left it blank and that was ok, as long as the company had some details about you.
