Car Insurance - no claims bonus removed

Hi, only place I could think of to ask this.
A colleague in work is just renewing their car insurance and they have discovered that the no claims bonus of over 5 years is now back down to zero.
They did have a bump last year, which even though it wasn't their fault and they didn't want to claim (excess more than the work), they still contacted the insurance company incase the other driver did, which they didn't.
My colleague gave the insurance information about what happened still reiterating that they didn't want to proceed with a claim, but the insurance company still contacted the other driver.
In the meantime my colleague got rid of the car (no work had been done on it) told the insurance company this, and nothing else happened until they got their renewal letter, basically insurance has trebbled.
I have already told her to write to the insurance company explaining what happened (not the details about the accident) and see how far she gets, as in my eyes the insurance have not actually processed a claim. Certainly they have not made any payout.

One claim doesn't "cost" 5 years NCD (normally 2).
So maybe this is an admin error???0
