Operation - query re BUPA

My OH is due to have an operation shortly (abdominal surgery) and the consultant says that although he hopes he can do a laparoscopy, it may well end up having to be a laparotomy.
Does anyone know what happens in this situation? Do you have to get authorization for both types of operation?
Presumably this is a scenario that can happen in any operation and so surely BUPA must make allowances for this.
Any reassurance greatly appreciated.
