What is the role of a care manager?

Last week a new care plan was arranged in order for me to better manage my illness.

There was a lot to take in at the time and one thing I'm not sure of is what role a care manager is going to take. Does anyone here have a care manager, who would know what their role will be?

I have a district nurse linked to my care and an OT and they have been able to sort out things that would make my life easier, so I don't know where a care manager fits in to this :o

    We have a care Manager for my wife. She is our community OT. I'm afraid I really cant help you though because i'm a bit clueless to the official role. We go to her for OT related stuff and that's about it really.

    Thanks, Oliver.

    I already have an OT, she's currently sorting out things like a key safe, hospital bed, etc, so I'm not sure what role the care manager will take.

    I will discuss it at the next visit, to get a more defined purpose.

    The Care Manager is usually a social worker. Their role is to co-ordinate between you and other agencies as to all your care needs.
