AXA Travel Insurance/Lloyds Bank Cust-They have lost declared conditions - BEWARE

I declared my medical conditions to AXA via my Lloyds Bank Account free travel insurance in 2006 by phone this included a triple heart bypass, they informed this was all what was required. The next holiday I confirmed my records over the phone and they told me I was not required to contact them on each trip and my records were up to date.
In September 2011 I suffered an angina attack in Cyprus and was taken to a private hospital.
AXA have just declined my hospital claim/bills based on the fact that in 2009 they sent out a letter to everyone asking people to contact them with any conditions and declare it to them because they were not going to take any of the records over from their old systems to the new one. I never received such a letter. They also claim to have no previous records including voice recordings of conversations I have previously had with them.
Has anyone else received such a letter asking for them to re-declare their conditions?
I am now dealing with the FOS regarding my claim.
Any help/advice appreciated.
