Debit card refund - machine, human or company error?

Hello all.

My wife paid for a lunch at a pub two weeks ago. She paid by debit card but an error by the machine or the waitress meant she was asked to re-enter PIN. She was charged twice. She went back to the pub last week where the manager tried to do a refund. Upon checking, the amount debited again (pending). The manager has contacted the PIN machine people 'Elavon' who said the amount would not be taken but it has.
The manager said that the two duplicate charges would be replaced but it would take 10 days.

Can anyone advise who is to blame? Are the staff not using the PIN machine properly, can the manager just give a cash refund, is 10 days acceptable?

    It will take 3-10 days for the refund to come through. They usually refund by the method paid so they will get their merchant charges refunded. The error can be caused by operator, machine or aquirer.
