I agree that talking to work is a good idea. Then they will know what hours you are doing and the impact it is having on you.
If, having formally told them about the impact which covering for your boss is having, you then become more unwell and do need time off with work-related stress, it's more serious for them. As another poster has said, they have a duty of care towards you - which includes taking all reasonable steps to avoid work-related stress. Once on notice about your situation they cannot say they did not know.
Good luck - and make sure you are keeping a record of all the extra hours you are doing. Are you entitled to time off in lieu? Can you ask for it?0
