NHS emails will be the same as most large organisations. You can set thresholds on mailboxes saying what size they can get to, what to do if the size is exceeded and when to send warning emails. The last resort is preventing you from sending emails - it's harsh but from an IT perspective it's like a proverbial kick up the butt that users need
If you backup/archive your emails, you can allow Outlook to always have the backup file open all the time (it's shows as a folder much like Inbox, Sent Items etc). This way, you retain your old emails but reduce the size of your mailbox on the server thus allowing you to send emails again.
And there endeth the sermon from an IT bod after too many years explaining to users why they can't let their mailboxes grow exponentiallyIn your case though, I'd make an exception :T

I've just informed Westie that my flat is not a Youth Hostel as she dumped her belongings all over the place. She doesn't seem to realise how much this bugs my Autism. I'm all on edge and I can't get comfortable.:(

Try asking your IT department how you can backup or archive your emails. There should also be an option to empty deleted items when you exit Outlook to save you doing it manually:
