PPI claim form from Northern Rock

Hi, sorry if this is in the wrong place, but couldnt see where else to post it.
Today i received a letter from NRock telling me i could claim against my PPI, and they sent me a form to fill in for me to highlight the reason of my claim.
1. I was not aware that the insurance policy was optional and i did not have to include it to obtain the loan.
2. I was not aware that the total cost of the PPI policy was added to my loan and interest was charged throughout the term
3. I was not made aware that I would not receive a proportionate refund of the cost of the PPI policy if i cancelled it before the end of the loan term.
4. I was not made aware of all significant exclusions, affecting my personal circumstances that could lead to a claim being declined
5. Any existing PPi policies or other means i had of making the monthly loan payments were not taken into account.
6. I was not asked sufficient details about my personal circumstances to allow the advisor to provide appropriate advice.
7. I was unduly pressured to include PPI during the sales process.
8. I was not aware that i was being sold a PPI policy with the loan.
Which one do you think i should tick to make a successful claim. I was in such a state when i took out the loan that to be honest i cant remember much about it.

I went with 2 & 3 although i havnt had a reply back from them yet so couldnt comment on whther its been successful or not.
