Help with improving PC hardware

Hi, i recently bought a PC for gaming and it sucks, i need to upgrade my graphics card and PSU and have a budget of £300-£350 for both items.

Please could you suggest a card and PSU in my price range which will improve my PC's gaming capabilities.

My current PC's specs are: AMD bulldozer FX-4100 3.6GHz Quad Core CPU
8GB 1333Mhz DDR3 RAM
1TB Hard Drive Storage Space
ATI Radeon HD 5450 1GB Graphics Card
High Performance Gigabyte Motherboard
DVD Re-Writer +/-
450w PSU
Windows 7 Home Premium 64-Bit Edition

Some 1600MHz RAM might help.
