PPI offer letter help!!!!!

I am looking for some advise about my PPI claim. I followed instructions as per Martin’s website and I was very pleased to receive an offer from the Halifax building society. However I am a bit confused about the contents of the offer letter.
The calculation they have stated is straight forward, but the letter mentions that I might have attempted to make a claim against my PPI policy in the past which was rejected, and if I am concerned whether the offer is less that the amount of money I would have received if the claim was accepted, they may need more information. They also say in order for them to take this into account I would need to complete a previously declined claim form, which they attached.
Has anyone received this form with their offer letter and if so, what was the outcome. I don’t really understand why this form was sent out, as surely the bank would know if there had been a previous claim and as the loan was over 10 years ago I cannot say for certain if I made a previous claim. Any help would be greatly appreciated.

This is just to let you know that you may have put a claim in on the policy previously and if it was rejected then the amount you may have been able to claim could have been more than the total PPI amount.
As the PPI only covered you for 12 months, it is a safe bet that you get more from the redress than you would if you had claimed on the policy.
Just send the acceptance back and put you did not make a claim that was rejected.
The redress they have given you will be the whole PPI amount and the 8%.0
