Trying to decide on the best offer

I have several 0% offers on the table and I am trying to work out which is best.
0% until June 2013 @ 5% fee + 2.2071% Per Month
0% until Nov 2012 @ 3.9% fee + 26.9% Per Annum after (sorry, don't know how to work out monthly rate from this)
On the face of it, the longest is obviously the best, but with that comes a higher fee and (I think) a higher residual rate at the end of the promotional period. Contrasting that, I am being offered a shorter period but with a lower fee and a lower post-promotional rate.
Neither of these offers is enough to take my whole balances, so I guess I need to maximize the best offer on the table with as much as I can from the worst of my remaining cards.
How can I work out which offer is best? I'm happy throwing together spread sheets but what maths should I use?

I would take the november offer first then do a BT in november to the June that way you dont lose out other than fee.0

2.2071% per month is equivalent to 29.95% APR
26.9% APR is equivalent to 2.0% per month0 -
to work out the best offer requires knowing how quickly you will be paying off the balances0
