Travel insurance: do I get the same medical exemption conditions if I wait to renew?

In a nutshell, I've got an Amex travel insurance policy that covers our various medical foibles, that's about to expire.

They've offered us a renewal that starts almost straight away that continues our coverage, but we're not planning on going anywhere for six months.

How likely is it that if I don't renew, and then call Amex after six months, they'd offer the same insurance policy that covers our various medical foibles? The salesperson on the phone wouldn't tell me one way or the other, for obvious reasons...

Phone around some other companies and get a quote on how much it would cost to insure you and if they cover your pre-existing conditions. That will give you an idea how easy it would be to get a replacement policy in 6 months. If you struggle then keep the coverage going if you expect to be going abroad regularly after six months.
