Statutory sick pay and SSP1 Form

I have been signed off work and would like to know what happens if my employer doesnt pay me any statutory sick pay? And if he does not provide an SSP1 form, the longer i wait for paperwork the longer my claim is delayed, is there a way of claiming support if i cannot get the necessary form filled in? Or can i fill the form in myself on behalf of my employer?
I know how my employer works and he is very slow off the mark to do anything and has a very i couldnt care less attitude.

If you are not you could print the SSP1 form from direct.gov and ask your employer to fill it in. But it is your employer who needs to fill it in to say why they won't be paying SSP and sign it.
