Creating certificates from powerpoint then embedding

I have written an e-learning package in powerpoint. At the end I want to embed a certificate into the program where the individual can complete their name and print it. I have created the certificate in powerpoint from the templates they offer, but this does not allow the name to be added. I have also tried to do it by creating a form but cannot find a way for them to enter a name.
I am using word 2007, but can also use 2003 and the program will predominantly used in 2003 as well.
