Worst I can do filling out the PPI form? - Please help

I am sorting out my dad's PPI claims but the paperwork he has is not in any sort of order. I do have account numbers and names but there are some of the things on the form that he is unsure of, such as exact dates they started, how they were sold to him and what they were for, i.e. loan, credit card, etc. and I know he won't take the time (and money) to ask the lenders for the information.
Do we have to fill in every last detail of the form or have there been success stories of people that have actually ticked the "don't know/can't remember" boxes? I presume you can write notes on the forms too?

Just answer what you know and say you do not remember for questions you do not know the answer to.0

If you answer questions with guesses or made up information then you actually stand more chance of failure than success. If they have evidence that shows the answers are wrong (at worst lies) then all credibility is lost and any balance of probability decision goes against the side with no credibility. There is an expectation that some things will not be remembered. Indeed, complaints that indicate a perfect recollection of events all those years ago are often create doubt.I am an Independent Financial Adviser (IFA).
