    FSA ICOBS states that "caused or contributed to the claim" is only relevant for breach of warranty/condition. This is not a breach of warranty or condition, it is a breach of utmost good faith. In such cases the FOS has published case notes which confirm that if the non-disclosure was inadvertent, and if the insurer would not have accepted the risk had it known the true facts, the insurer is entiteld to void.
