a. Your current UK address. Your current UK address so that Kalixa can verify your identity at this address. Do not use PO Boxes, work addresses, or halls of residence. No credit checks are required.
b. Your daytime phone number
c. Your Debit or Credit Card (for making an initial load onto the card)
4. Once your application is complete and has been successful (it will mean your ID and address have been verified), please email us and we will advise the Kalixa Prepaid Team that you have completed the application process and pass on your name and email address – so that they can place your winnings on to your new card.
5. If your application is successful, then your new card should arrive within 5-7 days with £10 spend money loaded on it (in addition to any money you have loaded as an initial load onto the card)
6. If you fail the application process, it would mean that we have been unable to verify your ID/address and you cannot redeem this prize.

Apart from the fact that it's for only £10...
Why should we give our CC/DC when they are supposed to give a card with £10 credit on it? Good Luck but I'm out of here.Thanks to those who help us to win !
