Hi Sue - how long ago was the tribunal?

Thanks again for your response, you seem to be the only one replying to me and I really appreciate it.
My appeal was heard in October 2011. I phoned this morning and was told I should have received a copy of why I was given the award at that time, however, the lady will email the relevant team and get them to send me a copy. Fingers crossed.
I have been sent form DLA434 to complete, would have been easier doing it online and I have tried to download the latest version October 2011, (well that's the one they sent me) but there isn't currently an electronic version of it available. So here goes for hand cramp for the next couple of days lol.
Also, I phoned my hospital and asked for a copy of an MRI report that was taken last month (proof of deterioration) but they state they don't send copies to patience....surely I have a right to this information. I just wanted to attach it to this form. I know DLA will probably contact my specialist and that's fine, I just thought it might save a little time.

Hi Sue,
To get copies of your medical records you will need to apply and perhaps pay http://www.nhs.uk/chq/pages/1309.aspx?categoryid=68&subcategoryid=160
They don't give out results to patients in the way you are anticipating so the above is the only way. They will likely contact your consultant anyway, but if you think attaching parts of your medical records will help, then apply for them.
They will not include actual "scan" pictures though, only letters to the doctor from consultant giving their opinion. I hope this helps“How people treat you becomes their karma; how you react becomes yours.”0 -
How helpful is your GP? Mine printed off a copy of my MRI report from the hospital website for me.
In case you should ever need to know, you can get a copy of actual XRay and MRI images from the hospital. They are provided on CD and there is a charge of up to £50 (although many charge much less than this). I got mine for £10 when I needed them for a 2nd opinion.0 -
Thank you both for the information. I was told by the xray department that I could pay £10 for the disc but she made it sound like it would be copies of the xrays only, maybe I mis-understood.
I will just send the form back, get it moving and let them request the relevant information. I was just trying to speed the process up, we all know how long it can take.
The consultant I see will be happy to write a report for me, this will include feedback from the MRI scan. I will wait and see if DLA will request any of this information.
It has taken me 7 hours to complete the form today :eek: I feel like I have done a full shift and now feeling the pain, just hope it was worth it.
