Answers my question perfectly! I always like to check the questionaire, etc where possible before visits and i couldn't work out where the actual questions were

That is why I always look at the print version, it expands to the full questionnaire.0 -

Did you get the 2 in town? I got one in Radford I can do on way home. When you first look it seems theres about 3 questions. But when I printed the questions you can see that there are ones relating to the gaps and cleanliness etc. Its still only 23 Q's and most are yes and no.
