    Answers my question perfectly! I always like to check the questionaire, etc where possible before visits and i couldn't work out where the actual questions were :)

    That is why I always look at the print version, it expands to the full questionnaire.

    Did you get the 2 in town? I got one in Radford I can do on way home. When you first look it seems theres about 3 questions. But when I printed the questions you can see that there are ones relating to the gaps and cleanliness etc. Its still only 23 Q's and most are yes and no.

    Depends on if you want to say hi or "take care" of the competition.
    This was 6 months out of date so I've changed it.
