ESA questionnaire help needed

hi ive received a questionnaire from the ESA :eek: am just wondering if anyone as any tips please.....
im 29 i have RSD (which is chronic pain 24/7) in my right hand arm and shoulder im on numerous medications daily and also have depression (not bad)
i have till the 14th of may to complete the form am crapppin myself because ive read quite a few stories on here about other peoples experiences and i dont seem to stand a chance
im in constant pain all day every day my RSD is horrible its like having petrol poured over my skin on my right hand arm n shoulder then setting it alite then having my limb incased in ice all at the same time even taking a shower is like having shattered glass poured over me not water it hurts so so much its unreal theres so many symptoms i get with my RSD like swelling skin tenderness etc etc etc
medications arent helping my gp no longer knows what to give me so he as asked me to try n find out off other RSD sufferers what medications may help
anyway any tips would be appreciated is it best to send off as much information as i can about my condition
i havent a clue where to start with my form

Fill in the ESA50 form as fully as possible... and have at your side the descriptors so you get additional direction as to relevant information... using the additional info section to cover relevant descriptors that the form doesn't really prompt information in relation to.

Supply as much supporting medical evidence as you can if possible... regarding your conditions and the descriptors.
