Life after an IVA

I'm hoping that somebody out there can help?
My husband and I entered IVA'S 5 years ago,we were told that we had to do 2 separate IVA'S. Mine went through straight away paying back 100% in the £ but my husbands failed so the ISP gave me the numbers and I had to contact them myself at the same time my hubby was undergoing tests for cancer.Eventually when I spoke to them all and asked them to return proxies ASAP it was agreed again 100% in the £. My hubby was diagnosed with cancer and given only 3-6 months to live his cancer was inoperable but they tried a new treatment from USA and he's survived almost 5years.
My IVA was cleared within 3 years but my hubby's has just completed and was renegotiated at 62p in the £.
At the start the ISP never said we had to close our bank accounts but our debts were not with the bank,she also said we could keep a credit card card each which we kept the ones with our bank and my hubby kept his Amex which he'd had for over 10 years.
Last week AMEX cancelled his card due to his IVA which is now cleared they stated he had gone against their terms and conditions and the balance was transferred to a debt recovery agent.
My hubby also had a business account with an overdraft,they are now requesting its paid off in full and if we can't they will close our accounts with them and the 2 cards we hold too.
Our mortgage is with the bank and we have a lot of equity so we asked the bank to add it to the mortgage but they have refused.
It seems that now we have completed the IVA'S mine over 2 years ago and my hubby's last December we are now being penalised, we have never defaulted on our mortgage or cards.Prior to the IVA'S we had always maintained minimum payments on all the cards in the IVA'S but we were beginning to worry about it my hubby being ill and I'm disabled.
Please can anybody offer us some advice what can we do about this we feel that we've done everything to clear the debts and now it's finished we feel we're being penalised?
