    When I got an ESA50 I made photocopies to fill in in pencil then took what I'd written to my local CAB and asked someone to check it over. Also, I answered ALL the questions even if the form implied it didn't apply to me. I wish I could remember how long it was before I heard anything but I was placed in the support group without any medical so I may have been fortunate.

    Before I sent the form off I scanned my responses into my PC in case I had to complete another one in the future.
