The hospital didn't give me one last time I was in, but I was in for less than a week.
I'm sure they wouldn't give you one as an outpatient, but as you can now self-certificate for up to 7 days that's not such a problem.Signature removed for peace of mind0 -

I wasn't attacked at work. I've checked my contract, I self certify for 3 days, still not sure if there's a form at work for this. I've got a sick note from the hospital for 10 days off, this is because I need an operation to fix my nose, I may need 2 though. I'll ask my boss about self certification when I drop my sick note in. He's thought I was faking it, until I told him to come round and see my injuries, he definately believes me now. I've looked into compenstaion and I think I will be eligable, but I want to wait a bit. I've got to go to the police station later this afternoon to see if I can identify the woman who attacked me.

Hospitals can issue sick notes, when I was discharged from my ankle op they gave me a two week sick note to take me up to my outpatient appointment. Then when I went back as an outpatient they gave me another sick note for 6 weeks to take me to my next outpatient appointment.
