Finding out your HC2 form number

I recently had some dental work done and although I've previously used my HC2 form with the practice they asked to see it again, which I know isn't unusual. However I have left my certificate at uni on the other end of the country so getting it is a very expensive train journey away. Is there any way I can retrieve the number on the certificate by ringing the NHS etc., as the practice said this would suffice?
