New Lloyds PPI Claim

My partner would like to reclaim her PPI on her credit card.

Reason being she didn't even know she had it till we caught eye of it on a statement this month!

This makes us look bad, as it was taken out in September 2008!

We have looked at some old statements and seems the monthly charges have been between £8-£10 pounds.

She went into Lloyd's to explain and they told her she could reclaim it and gave us this form;

but to me that looks like they are trying to find every possible way out of replaying depending on what you answer?!?

She applied for the card in branch, and can not remember even being asked if she wanted or even needed it to be honest.
