Tipping in Canada/US

I know tipping is a big thing in the US, especially in restaurants etc. We are off to Canada in June and we expect to be tipping at restauants etc but I am big less clued up about tipping in hotels. Who do we tip (if anyone?) and how much shoudl we tip them?? Is not easy to just say 10-15% as applies to restaurants.

pretty much anyone providing a service
my personal amounts are
taxi drivers 5-10%
taxi callers $1-$2
Bellhops $2-$4 per big bag
Waiters 12- 20%
Bartenders $1-$3 per drink
Fast food staff $1-$2 (in jar on counter ect)
Maitre de $3-$10 ( depending on seats you get)
try a discreet tip of $20 with hotel check in staff for a room upgradeBetter in my pocket than theirs :rotfl:0
