Change of name incompetence

A story of incompetence.
My sister got married last summer and decided to change everything into her new married name. Simple enough surely.
Two credit card companies and her bank sent her new replacement cards as requested but without bothering to change the name on them. She had to send them back and ask for new replacements.
Then her bank sent her 2 debit cards, 5 cheque books and 3 paying-in books on the same day, all issued with slightly different variations of her new name. (Example. If her new name was Mrs A B C Smith, one would say Mrs A B C Smith, one Mrs A B Smith, one Amanda Smith etc). She called them and asked them which to debit card to keep and which to cut up. They said either. A month later the card she kept stopped working. When she called them they said it had been cancelled automatically because it wasn't the latest card to be issued; the one that she cut up was.
One of her credit cards expired earlier this year and she duly received a new replacement card for one of the replacement cards in her new name which had been issued a few months before. She recently travelled to Spain, a country which she visits very frequently, where all chip/pin terminals print the cardholder name on the receipt. I have seen the receipts. The name being printed on the receipts is her old name which does not correspond to the name printed on the front of the card. She is very concerned about this as she is worried that she will be accused of using a 'dodgy' card.
Any suggestions as to HOW this is happening? Everything that happened last year she found quite amusing but this latest business with the name on the receipts has got her worried.

Ben as long as the name is correct whatever the format it is fine to use.
If the name is wrong then I would get her to phone the bank and complain.
I too have an unusual name and it has taken a few goes. However I have cards which show my name in very different formats

If its wrong she could be hauled up on it, chance is very little. Prime example I use my card all the time in Asda, I use my husband card in asda and the name is on the receipt. In 5 years never been touched or challenged once!0 -

Thanks. The thing is the name is different between the name printed on the card and what prints out on receipts. For example, if the name printed on the card was Angela Smith the name on the chip which prints on the receipts is Angela Jones.
I agree that the chance of her being pulled up on it are remote but in Spain it is normal practice for ID to be requested when using a card. I think she had visions of some very observant person noticing one name on the card and a different name on the receipt and calling the police or something.0
