Just to get back on track...!! as an ex-IT Manager I have seen plenty of systems fail but very rarely, if ever, had a full confirmation printout only to find at that endpoint that this has disappeared from the records database. That does show a very poor system if database records go missing after completion and/or connected systems don't pick up on shared data.
