requesting money from paypal

i now have a premier paypal account.
i wish to withdraw some money,they are asking for bank account details.
my problem is they ask for the names as they appear on my statement which is similar to this j p smith.
it asks for my first name but states you must put whats on the statement.
so do i put my first name being john or j p.
when you view the read me it states it must be the name they have on file which will be john but the statement, chq book and debit card all have j p smith.
any advice will be great thanks.
