Reclaiming PPI that I didnt ask for!

When I originally applied for my Nationwide credit card, I ticked the box that said NO, for PPI and Card Protection Plan. Yet hearing all the fuss about PPI, I thought I'd check my statement. Lo and behold I've been paying PPI all this time. I have a copy of the original application form, (from Nationwide). What is my best course of action? Would a letter be adequate, or should I fill out the standard form? Thanking you in advance.

Thank you for the reply. I thought it was only for being mis-sold PPI not being given something you expressly asked not to have.0
