Duplicate online transactions

Hi all. Twice now with different companies, I've had a transaction that showed an 'error, unable to complete the transaction' message on screen. Only to find later on my CC statement that the transaction DID complete and because I did it again - because of the error message - now have two transactions. One of these was for over £540 (airline tickets)! Eventually I got both transactions refunded, but has anyone else had this problem, or is it me?
