How to claim back PPI from DFS (Hitachi Capital)

Looking for a wee bit of advice regarding claiming back PPI I didn't realise I was paying on sofas I bought back in 2006 from DFS.
I was sold '5 Year Leather Care' by the salesperson as part of the deal but no mention of PPI at the time. I was talked into this cover as I was informed my home contents cover would not cover various claims I may have needed to make on the sofas. As the payments were completed in 2009 I no longer had a copy of the loan agreement however I wrote to Hitachi Capital who have now sent me a copy. On looking at the agreement I discovered a box ticked at the bottom corner for PPI costing £6.85 per month over 24 payments.
Should I now be completing the online questionaire and should I be sending it to DFS or Hitachi Capital??
Having read a few other posting from persons claiming direct from DFS i noticed that DFS were demanding bank statements as proof of payment. This will be a problem for me as I have changed banks since completing the full payment of the loan in 2009 and have no statements. I do however have a copy of my credit report which shows the loan was settled in July 2009 so not sure if that will do as proof?
Any advice would be much appreciated!
