Want to reclaim PPI but don't even have account numbers !

I looked at the PPI letter template asking for terms and conditions but that has space for account numbers. I do not even have these. Can I still use the template and say when loans (roughly) were taken out and ask for details to be sent ?

Contact your lender or use Credit Expert or a similar site to obtain all the details on your credit history.0
