1st time claiming PPI, newbie question!

Hi there
I have a couple of questions that I'm hoping someone can help with.
My bank is Lloyds TSB and in the past I've had a few loans with them. Never thought I'd be eligable to try and claim the PPI back as I thought they were too far back. However, after combing through all my statements online I have managed to narrow down 3 of them to the date that the money was transferred into my account. The earliest being taken out in June 2005 and payed off in June 2006. One in March 2006 payed off in April 2008. The last one from August 2008 'til September 2009.
The thing I don't have any paper work for these loans. Now I know you can get the template letter and send that off to get the credit agreement and the T & C's, but it needs the account number, my question is will the loan number that appears on my statement be enough? As it's currently the only thing I have to go on. Would that be the account number and if not, will they be able to use the number that appears on the statement?

ETA: Would the date that they transferred the money into my account also be the date that the agreement started?

Should be enough, Put the account number you have on the form and ask the Bank to look at the rest of your accounts. They should be able to find the others.0

Does that mean I can just go straight to the complaints form then as detailed here http://www.moneysavingexpert.com/reclaim/ppi-loan-insurance#step3,fill that in and send it off to Lloyds, or have I got that bit wrong?
Should I request the credit agreement and the T&C's first?
