Letter advice

Hi I am just after a bit of advice if its out there.
I am in the process of trying to reclaim the PPI i paid on a Sainsburys loan, I have read the advice on the website and I have downloaded the form and filled it in. Thats as far as ive got though as im not sure whether i send the questionaire which ive filled in off on its own or do i write a letter to go with it. I am also unsure who i send it to (ie what department) :think:
Any help would be much appreciated.

You do not need a covering letter, the address is.
