PPI re claim from hsbc and dfs

Hi Im all new to this and think im due some money. Please can you help me out with what forms i need to fill out and send to my bank. is is just the Ombusman questionaire that i send to me bank?

Also do you know if i can claim back the PPI from DFS for the sofa that i purchased?

Then fill this form in,it is number 3,then send it to the bank.

Make a copy of this in case it has to go to FOS.
