Abbey Credit Card PPI - Who to contact?

I've just been digging around for paperwork relating to my RBS loan PPI claim, and found 5 statements from when I had an Abbey Credit Card.
The 5 statements are from the account opening in Feb 2007 with nil balance and then the balance transfer for £3,324 until me closing the account in June 2007.

I clearly remember applying for my RBS loan to clear my Abbey credit card because I was shocked at the PPI added to my account. I think I must have applied online for the credit card because I have never banked with Abbey and wouldn't have gone into a branch. I don't know if I would have missed a pre-ticked box when applying.

I can see I only paid 3 PPI payments; £27.06, £26,67 & £0.25. I know it's only £53.98 but it's £53.98 I begrudged paying at the time so much I sorted it out with a loan.

Please could someone kindly tell me who I should contact to claim the PPI back as I know Abbey no longer exist and I don't think it'll be just a quick freephone call as I did with RBS.

Thanks in advance.

I've filled out the questionnaire and photocopied my credit card statements... I guess I just need to figure out where to send them all to.
