Mbna card ppi

Right then , recently i wrote to mbna complaining about ppi on my virgin credit card saying i shouldn't have been sold this due to been self employed , after filling in the questionnaire they are saying that i took out the policy online and that id said i was employed , now i don't remember this as it was back in 2006 . They are saying that the matter is closed and if i want to take this matter further i will have to fight my case with the ombudsman .
i have downloaded the questionnaire but before i fill it in wanted to ask for a bit of advice ...
1.. do i need to provide them with proof of self employment ?
2.. i took out finance for some furniture at the same time (2006)and still have the original agreement where it shows clearly that i ticked the self employment box , i was also sold ppi and am in the process of claiming that back too, would it be worth photocopying that and sending it to the ombudsman with my claim to back up my argument that i was self employed and that in no way would i have said i was employed to the mbna?... or am i best not disclosing it ?..i`m really nervous in case i mess the complaint up and end up getting nothing due to filling the form in wrong even thou i know i`m in the right if that makes sense
3 ..once the ombudsman makes his mind up is that the end of the matter or can you challenge his decision too?
... 4 is there a part of this board that gives advice on filling the questionnaire in ? any help would be much appreciated..thanks in advance

    They were the first to respond to me - they shoved a cheque in the post for £x000 compensation and 3 x £x000 interest!

    The problem you have is that they have documentation which indicates you did tell them you were employed. You may have done this in error but they are entitled to rely on what you told them.
    3 ..once the ombudsman makes his mind up is that the end of the matter or can you challenge his decision too?

    If you dislike the decision by an administrator then you can ask an Ombudsman to consider it (as can the firm).

    If you dislike the decision by an Ombudsman, you can go to court - but will lose the right to reinstate the Ombudsman's decision if the court is less favourable.
