Reclaiming PPI from Cahoot

I have had a Cahoot (now Santander) flexi loan since 2001. I think i was mis-sold PPI at the time. Do i contact Santander with respect to this?
Has anyone had any joy with Cahoot / Santander claims?
Also, is anyone aware of a standard letter i can send reclaiming the PPI?
