Urgent help needed with Lloyds TSB

I started a new thread about this last month here it is https://forums.moneysavingexpert.com/discussion/3759283
In a nut shell, claimed for a loan I had in 1997 for 5 years with Lloyds got a upheld letter saying they are paying me, rang up to check and it wasnt for the loan I thought it was it was for a loan in 1991 for 24 months, went into branch they told me both my loans had the same account number (apparently TSB used to do this) and sent off fresh claim form via fax from the bank because over the phone they said they couldnt locate my 1997 loan
rang Lloyds PPI today as I received my payout for the 1991 loan just to confirm my other claim and they turned around and said they havent got another claim for me and could I send them copies of original fax from bank and list of users accounts held with Lloyds that I got from the branch because they cant see my 1997 loan.
Im just on my way to my branch again for them to send the information off with there internal mail but should I be worried about this or should I contact FSA or FOS?
Ive also asked for a SAR and im just waiting for that to arrive.
BTW sorry for long winded story :(

After a phone call on to Lloyds on thurs 1st March chasing this up (to be told nothing had been done) I received a letter today date marked 2nd March saying complaint blah blah blah then near the end it says "Following your most recent letter received I have checked our records and can confirm you have only taken out one loan which had insurance attached" and "after further investigation I can confirm that loan number ************* was taken out on 24 april 1997 and we have already addressed this loan"

So I rang the number and asked why when I got my upheld letter in Jan 2012 I was told over the phone it was for the a loan I had in 1991 for 24 months (hence the low amount) and they didnt know anything aout my loan in 1997 (for 60 months) so why did Mr Hobbs from PPI cust services say I only had PPI on my 1997 loan? no answer and I couldnt speak to Mr Hobbs so I told them I will contact the FOS and hung up.
Spoke to the FOS and made my complaint, this is getting bigger than War and Peace :) but very very frustrating.
Ok I might of only had PPI on my 1997 loan but why pay me out on my 1991 loan? and vice versa but saying that the local branch have said you had PPI  im going crazy.....
