Policy T&C's?

Just starting out to reclaim PPI on old mortgage and potentially on credit cards.
I have obtained my credit report from Equifax and it details all old credit agreements from last 2 addresses.
I've also downloaded the template from MSE in order to obtain a copy of the orginal policies from the lenders as I don't have these to hand anymore.
My question is, the report only indicates the last 4 digits of the policy number. Is this sufficient to go back to lender with for a copy of the policy agreement, bearing in mind that I will of course be proving my personal details, old address etc?
Also, if I was paying for additional insurance on credit cards, would this have been billed seperately on my monthly invoice or built into the monthly payment. Assuming that I have only ever used a credit card for the interest free introductory period, I have always cleared in full each month and so do not remember paying anything additional. Would it be fair to assume that I wasn't paying for additional insurance in these instances?
Any advice would be appreciated.
Thank you.

The last four digits are of no use to the lender. However, your name, date of birth and address (and past addresses if applicable) will be.lso, if I was paying for additional insurance on credit cards, would this have been billed seperately on my monthly invoice or built into the monthly payment.
it appears as a transaction just as any other transaction does.Assuming that I have only ever used a credit card for the interest free introductory period, I have always cleared in full each month and so do not remember paying anything additional. PPI was charged on those that borrowed. Not those that cleared each month.I am an Independent Financial Adviser (IFA). The comments I make are just my opinion and are for discussion purposes only.
