Jobs that have been taken being advertised?

I come across this a lot. Had it recently with an agency....

advertised a job
I applied
They called about said job but I was busy so missed the call
Called back within minutes, person had popped out - left message to call back
Email 10 minutes later to say not being put forward for the role
Called again asking why because I had been called about the job so why now not being put forward? - it's been filled internally so job withdrawn.

Had it so many times, jobs advertised that are already filled usually within the company. They don't have to advertise a job I know but if they do, they shouldn't advertise ones that are not vacant.

I run a small business and a couple of times, i've agreed to put an advert in the local job pages and between that and its print date, had someone call in on spec looking for a job, interviewed them gave them the job, but left with an advert that couldn't cancel, and believe you get too many applicants to do it for fun.
