Why are tier thresholds not on our bills ?

They do.
The threshold is an annual figure eg. 720 kwh @ tier 1 for electricity over a year. You will not get a bill which covers a year however (usually), it is also unusual to get a bill covering a perfect quarter which would be 180 kwh @ tier 1 (using 720 annual threshold). If the bill covered 50 days you would be charged 98.6 kwh @ tier 1.
I have had bills from various suppliers and have always had this the same way.0 -
I note my npower bill, on which I had gas usage below the threshold does not specify the bands at all, just lists the lower rate one.0

It will always be on your bill if:
1) You are on a tiered tariff, and
